Project Assessment Checklist

A 30-question project assessment checklist a PM or sponsor can run in under an hour. Five categories, plain-English questions, and a simple score you can act on.

The five assessment categories

Each category has six questions you score 0-2 to produce a health rating

1. Scope clarity

Is what we are building defined, agreed by sponsors, and matched to a measurable outcome?

2. Plan integrity

Is the plan realistic, dependency-aware, and backed by defensible estimates?

3. Risk & RAID

Are risks named, owned, and reviewed on a cadence that actually drives action?

4. Governance

Are decision rights, steering, and escalation paths clear to everyone involved?

5. Stakeholders

Are sponsors, vendors, and impacted teams engaged on the right cadence and at the right depth?

Sample questions from the Project Assessment Checklist

  1. Is the project charter signed by the executive sponsor, and does it state a measurable outcome?
  2. Does the current plan account for every known cross-team dependency by name and date?
  3. Are the top five risks owned by named people with a next review date in the next two weeks?
  4. Has the steering committee approved any change to scope, schedule, or budget in the last month?
  5. Are status reports producing decisions, or are they consumed as information only?
  6. Do impacted business teams know what is changing, when, and what is expected of them?
